Remote eConsent - shortened Remote eConsent - shortened Please Consider This Research Opportunity Research is needed to make discoveries that improve the health of everyone, but only a small group of us have ever participated in research. To solve this problem, the Mount Sinai Health System is inviting all of our patients to participate in a historic research program – the Mount Sinai Million Health Discoveries Program (“Mount Sinai Million”). Mount Sinai Million will enroll one million Mount Sinai patients to better understand how genes and the environment influence our health. By inviting millions of patients of all races, ethnicities, ages, and genders to participate, we expect to make discoveries that improve the health of everyone. Please read the information listed below and the consent form that follows, then decide whether you would like to participate in the Mount Sinai Million: Participation is simple - it includes reviewing and signing this consent form to give us permission to use information already in your medical record, in addition to what will be added in the future. We will also use the leftover blood samples collected during your clinical care. The information and samples collected will be used in future research studies, including genetic sequencing, to make scientific and medical discoveries that will potentially help millions of people. Participation in this study will not affect your ability to receive medical care or any eligible benefits within the Mount Sinai Health System. Your participation is voluntary, free and you can stop at any time. You are not expected to get any benefit from taking part in this research study, but the research may lead to scientific and medical discoveries that will help millions of people.
